Is has been reported that pharmaceutical errors cause at least one death everyday day and injure approximately 1.3 million people annually in the United States.  An estimated 106,000 deaths occur every year from no-error, adverse effect of medication.  7,000 more deaths per year occur as a result from medication errors in hospitals.

In a study publish published in the Archives of Internal Medicine, drug errors occurred in nearly one of five dose in a typical 2000 bed hospital, which translates to about tow errors per patient daily.  Seven percent of the errors were considered potentially harmful.  The most common errors were giving patients medication at the wrong time or missing a dose.

The Joint Commission on Accreditation of Healthcare Organization (JCAHO) has set safety standards to reduce errors in a medical setting as pertaining to prescriptions.

Pharmaceutical injuries are sometimes difficult to identify. The damage may be caused by because two or more drugs interact improperly, or, the manufacture may have failed to adequately test its drug..
